Summary
[Employer hidden — view at passion-project.co.uk] Health Care Agency is recruiting a Team Leader to lead the Enable and Empower team. The role involves supporting individuals with empowerment, self-help skills, and life progression opportunities. The ideal candidate will have a Diploma level 3 in Health and Social Care or a willingness to work towards this, previous team management experience, experience of working with individuals with a learning disability and good communication skills.
- Supporting the Manager with the management, development, coordination, and growth of the Enable & Explore service.
- Developing a team of Support Workers, conducting regular supervision, appraisals, and progress reviews.
- Ensuring effective communication is maintained through supervisions, team meetings and handovers for the area responsible for.
- Coordinating training, induction, care certificate workbooks and observations.
- Support the Service Manager with planning staff need and usage for efficiency and service delivery.
- Driving high levels of staff engagement and morale in the team.
- Management of any people concerns, such as absence management, performance concerns. Managing team and resources in line with agreed budgets.
- Ensuring service delivery is co-produced with individuals we support, their families and other stakeholders.
- Ensuring that services users are always safeguarded through adequate risk assessments being undertaken for an individual.
- Work in conjunction with the Activity Lead, to provide an activity program for the individuals we support. Ensuring all new individuals, we support are provided with a thorough assessment and a comprehensive care plan is in place prior.
- Ensuring medication administration requirements and specialist health care tasks are identified, assessed, and administered following current legislation.
- Ensuring any safeguarding issues are addressed immediately.
- Ensuring all records are accurate, up to date and in line with legislation and Hamelin Trust Policies.
- Ensuring that the required risk assessments are in place for all relevant aspects of the service.
In order to be successful within this role you should have:
- Diploma level 3 in Health and Social Care or a willingness to work towards this. Full driving license
- Experience in managing a team.
- Experience of working with individuals with a learning disability
- Good communication skills
- Leading a learning culture
- ICT skills such as MS Office suite
# Team Leader - The role:
We are looking for a Team Leader to lead the Enable and Empower team to support the individuals who use the service with empowerment, self-help skills and life progression opportunities. Within this part of the service, our teams support individuals within a studio environment with ongoing progressive activities and working within the community.
The service has recently been redesigned to allow a broader and deeper focus on individuals having different experiences and opportunities, and with an experienced team of care professionals, and some newly created roles to help the service reach its full potential.
# In addition to this as our Team Leader you will be responsible for:
