Summary

- Clinical screening and ordering of outpatients, TTAs, and inpatient medicine.
- Ward visits to perform medication reconciliation.
- Assess high-risk medication and make clinical interventions.
- Support to ward-based nurses and doctors.
- Checking trial prescriptions.
- GPhC Registration Required
- Previous experience in an acute hospital setting is also desirable
